fa بررسی علایم عصبی در کودکان و نوجوانان زیر ۱۸ سال مبتلا به کووید19 بستری در بیمارستان شهید صدوقی در سال‌های 1399-1398 Evaluation of Neurological Symptoms in Children and Adolescents under 18 Hospitalized with COVID-19 at Shahid Sadoughi Hospital بیماریهای مغز و اعصاب Neurology پژوهشي Original article <div style="text-align: justify;">مقدمه: ویروس SARS-CoV-2 با ایجاد طیف گسترده‌ای از علائم، از جمله درگیری‌های عصبی، کودکان را نیز تحت‌تأثیر قرار می‌دهد. هدف این مطالعه، بررسی فراوانی و الگوی تظاهرات عصبی در کودکان مبتلا به کووید-۱۹ بود. روش‌ها: این مطالعه توصیفی–تحلیلی بر روی کودکان زیر ۱۸ سال مبتلا به کووید-۱۹ انجام شد که به اورژانس اطفال بیمارستان شهیدصدوقی مراجعه کرده یا از مراکز دیگر ارجاع شده بودند. اطلاعات دموگرافیک، علائم بالینی با تأکید بر تظاهرات عصبی، عوارض، و نتایج تشخیصی شامل RT-PCR و سی‌تی‌اسکن ریه ثبت شد. پیگیری پس از ترخیص با مصاحبه والدین انجام گرفت. داده‌ها پس از کدگذاری و کنترل صحت، با نرم‌افزار SPSS version 16 تحلیل شدند. نتایج: درمجموع ۱۹۱ کودک (49/7% پسر و 50/3% دختر) بررسی شدند. شایع‌ترین تظاهرعصبی، اختلالات حرکتی مانند عدم تعادل، ضعف، لرزش و درد اندام‌ها بود (14/9%). پس از آن، تشنج (12/2%) و سردرد و اختلال هوشیاری (هرکدام 8/8%) قرار داشتند. علائمی مانند سرگیجه، بی‌اشتهایی، خستگی و بی‌حالی در 22/1% موارد گزارش شد. در مجموع 48/2% بیماران حداقل یک علامت عصبی داشتند. بین بروز علائم عصبی و جنسیت، بیماری زمینه‌ای یا مدت بستری ارتباط معناداری مشاهده نشد؛ اما سردرد با افزایش سن شیوع بیشتری داشت (p<0.01) و اختلالات حرکتی در گروه سنی ۵ تا ۱۳ سال شایع‌تر بود (p<0.001). وجود نمای گراندگلاس در سی‌تی‌اسکن ریه نیز با اختلالات هوشیاری ارتباط داشت (p=0.01). نتیجه‌گیری: علائم عصبی، به‌ویژه اختلالات حرکتی و سردرد، در کودکان مبتلا به کووید-۱۹ نسبتاً شایع است و برخی از آن‌ها با سن و یافته‌های تصویربرداری ریه مرتبط‌اند؛ در حالی که جنسیت، بیماری زمینه‌ای و مدت بستری نقشی در بروز این علائم ندارند.  </div> <div style="text-align: justify;">Introduction: SARS-CoV-2 virus also affects children by causing a wide range of symptoms, including neurological involvement. This study investigated the frequency and pattern of neurological manifestations in pediatric patients with COVID-19. Methods: This descriptive–analytical study was conducted on children under 18 years of age with COVID-19 who were referred to the pediatric emergency department of Shahid Sadoughi Hospital or referred from other centers. Demographic information, clinical symptoms with emphasis on neurological manifestations, complications, and diagnostic results including RT-PCR and lung CT scans were recorded. Followed-up after discharge was conducted by interviewing parents. After coding and checking the accuracy, the data were analyzed with SPSS version 16 software. Results: A total of 191 children (49.7% boys and 50.3% girls) were studied. The most common neurological manifestation was motor disorders such as imbalance, weakness, tremor, and limb pain (14.9%). This was followed by seizures (12.2%), headache and impaired consciousness (8.8%). Symptoms such as dizziness, anorexia, fatigue, and lethargy were reported in 22.1% of cases. In total, 48.2% of patients had at least one neurological symptom. There was no significant association between the occurrence of neurological symptoms and gender, underlying disease, or length of hospitalization; however, headache was more prevalent with increasing age (p<0.01) and movement disorders were more common in the 5–13-year age group (p<0.001). The presence of ground glass appearance on lung CT scan was also associated with impaired consciousness (p=0.01).
