fa ارزیابی آزمایشات ادراری در کودکان مبتلا به کووید-19 بستری در بیمارستان اکبر مشهد طی بازه زمانی 1400-1401 Evaluation of Urine Analysis in Children with covid-19 Hospitalized in Akbar Mashhad Hospital during the Period of 2021-2023 اطفال Pediatrics پژوهشي Original article مقدمه: هدف این مطالعه ارزیابی نتایج آزمایشات ادراری در کودکان زیر 18 سال مبتلا به کووید-19 بستری در بیمارستان اکبر مشهد بود. تمرکز اصلی بر شناسایی عفونت‌های میکروبی، عوارض کلیوی و تغییرات در شاخص‌های ادراری این بیماران بود. <div style="text-align: justify;">روش بررسی: این مطالعه به صورت مقطعی بر روی 199 کودک با تست PCR مثبت برای کووید-19 که از فروردین 1400 تا اسفند 1401 در بیمارستان اکبر مشهد بستری بودند، انجام شد. داده‌های دموگرافیک و نتایج آزمایشات ادراری از پرونده‌های پزشکی و سیستم HIS بیمارستان جمع‌آوری گردید. آزمایشات ادراری شامل بررسی پروتئین، گلوکز، کتون‌ها، بیلیروبین، pH، نیتریت و سلول‌های خونی بود. تحلیل آماری با استفاده از نرم‌افزار SPSS version 16 انجام شد. نتایج: در این مطالعه ۱۹۹ کودک مبتلا به کووید-۱۹ بستری بررسی شدند. پروتئین و گلوکز در بیشتر موارد منفی بودند (به ترتیب79/9 % و 84/9%)، اما مقادیر کم تا بالا نیز در برخی موارد مشاهده شد. نیتریت و گلبول سفید در ادرار همگی منفی بودند  ولیکن هموگلوبین در ۱۹۵ مورد (98/5%) مثبت بود. کشت ادرار در برخی موارد نشان‌دهنده حضور باکتری‌هایی مانند کلبسیلا پنومونیه، استافیلوکوک اورئوس و گونه‌های کوآگولاز منفی با الگوهای متفاوت مقاومت آنتی‌بیوتیکی بود. تحلیل آماری نشان داد که پارامترهایی مانند رنگ، ظاهر، پروتئین، خون، pH، سلول‌های اپی‌تلیال، موکوس و باکتری با وضعیت نرمال یا غیرنرمال آزمایش ادرار ارتباط معناداری دارند (0/05 p<). با این‌حال، بین جنسیت  (0/373p =) و سن (0/431p =) با وضعیت آزمایش ادرار ارتباط معناداری مشاهده نشد. نتیجه‌گیری: این مطالعه نشان داد که عوارض کلیوی و نتایج غیرطبیعی آزمایشات ادراری در کودکان مبتلا به کووید-19 بستری شایع نبوده و بیشتر بیماران در وضعیت نرمال بودند.</div> <div style="text-align: justify;">Introduction: The aim of this study was to evaluate the urinary test results in children under 18 years old hospitalized with COVID-19 at Akbar Hospital in Mashhad City. The main focus was on identifying microbial infections, kidney complications, and changes in urinary indicators in these patients. Methods: This cross-sectional study was conducted on 199 children hospitalized with a positive PCR test for COVID-19 at Akbar Hospital in Mashhad City March 2021 to February 2023. Demographic information and urinary test results were collected from medical records and the hospital HIS system. Urine tests included checking for protein, glucose, ketones, bilirubin, pH, nitrite, and blood cells. Statistical analysis utilized SPSS software for execution. Results: This study assessed 199 hospitalized pediatric patients with COVID-19. In the majority of instances, protein and glucose tested negative (79.9% and 84.9%, respectively), although varying levels were observed in certain cases. Both nitrite and white blood cells in the urine tested negative. Hemoglobin was positive in 1.5% of cases, blood in 23.5%, and epithelial cells were positive in some samples. Urine cultures in some cases indicated the presence of bacteria like Klebsiella pneumoniae, Staphylococcus aureus, and coagulase-negative Staphylococcus species, displaying varying patterns of antibiotic resistance. Statistical analysis showed that parameters such as color, appearance, protein, blood, pH, epithelial cells, mucus, and bacteria were significantly associated with either normal or abnormal urine test results (p<0.05). However, no significant association was detected between urine test results and gender or age.
