fa بررسی ارتباط میزان بیان ژن COX-2 باعلائم کلینیکوپاتولوژیک و پیش‌آگهی در بیماران مبتلا به سرطان معده Investigation of the Relationship between COX-2 Expression, Clinicopathological Symptoms, and Prognosis of the Patients with Gastric Cancer جراحی Surgery پژوهشي Original article <div style="text-align: justify;">مقدمه: مطالعه حاضر جهت بررسی ارتباط میزان بیان ژن COX-2 با علایم کلینیکوپاتولوژیک و پیش‌آگهی در بیماران مبتلا به سرطان معده طراحی شده است. روش بررسی: مطالعه توصیفی-تحلیلی جهت دستیابی به اهداف تعیین شده در بازه زمانی سال های 1387 لغایت 1398 به بررسی بیماران مراجعه‌کننده به بیمارستان امام‌رضا (ع) شهر مشهد، ایران، پرداخته است. در این مطالعه اطلاعات دموگرافیک بیماران، علائم بالینی بیماران، یافته‌های مربوط به تومور، نوع جراحی و سطح بیان ژنCOX-2  (با استفاده از تکنیک Real time PCR) ثبت و بررسی گردید. داده‌های ثبت شده با استفاده از نرم‌افزارversion 16  SPSS و با بکارگیری شامل آزمون‌های مجذور کای، دقیق فیشر، ضریب توافق گاما، آزمون آنالیز واریانس یک طرفه و یا آزمون کروسکال والیس در سطح معناداری 0/05 مورد تجزیه و تحلیل قرار گرفت. نتایج: از بین 110 بیمار وارد شده به مطالعه، تعداد 55 بیمار، با توجه به معیارهای تعریف شده، مورد بررسی قرار گرفتند. در این بین 21 (38/2%) نفر از بیماران دارای مقادیر کاهش یافته بیان ژن بودند، در 18 (32/7%) بیمار بیان ژن در محدوده نرمال قرار داشت و در 16 (29/1%) نفر افزایش بیان ژن دیده شد. میانگین ماه‌های بقای بیماران پس از جراحی، 17/41±23/07 ماه به‌دست آمد. در مطالعه حاضر ارتباط معنادار آماری بین افزایش بیان ژن COX-2 و علائم بالینی، سن، جنسیت، بقا (بر اساس ماه)، پیشرفت تومور، نوع جراحی، ویژگی‌های بافت‌شناسی، سابقه خانوادگی سرطان و سایر موارد یافت نشد (0/05> P)؛ ولیکن در نظر گرفتن بقای سالیانه در دسته‌بندی سالیانه، ارتباط معناداری را با میزان بیان ژن COX-2 نشان داده است (0/028=P). نتیجه‌گیری: علی‌رغم عدم یافتن ارتباط معنادار بین بیان ژن COX-2 و پارامترهای بررسی شده در بیماران مبتلا به سرطان معده در این مطالعه، وجود ارتباط معنادار بین میزان بیان ژن و میزان بقای سالیانه بیماران از اهمیت به‌سزایی برخوردار می‌باشد.  </div> <div style="text-align: justify;">Introduction: The present study was designed to investigate the relationship between COX-2 gene expression, clinicopathological symptoms, and prognosis of patients with gastric cancer.</div> <div style="text-align: justify;">Methods: The present cross-sectional study was designed to achieve the scientific aims among patients referred to the Imam Reza Hospital, Mashhad, Iran from 2008 to 2018. In this study, patients' demographic characteristics, clinical signs and symptoms, tumor findings, type of surgery, and COX-2 gene expression level (using Real Time PCR technique) were recorded. The data were analyzed by SPSS software (version 16), statistical tests, including Chi-square, Fisher's exact, Gamma coefficient, one-way ANOVA, or Kruskal-Wallis test at a significance level of 0.05.</div> <div style="text-align: justify;">Results: Out of 110 patients included in the study, 55 patients were evaluated according to the criteria. Among them 21 (38.2%) patients had decreased levels of gene expression, in 18 (32.7%) patients gene expression was in the normal range, and in 16 (29.1%) patients increased gene expression was seen. The average survival months of patients after surgery were 23.41 ± 23.07 months.
