fa ارزیابی اثربخشی و عوارض جانبی کاتتر ویژه تلقیح داخل رحمی اسپرم در زوج‌های نازا، یک کارآزمایی بالینی تصادفی Evaluation of the Efficacy and Side Effects of Catheter-Specific Intrauterine Insemination in Infertile Couples, a Randomized Clinical Trial زنان و زایمان Obstetrics کارآزمایی بالینی clinical trial <div style="text-align: justify;">مقدمه: تلقیح داخل رحمی اسپرم (IUI) یکی از روش‌های کمک باروری است که احتمال قرار گرفتن تخمک در معرض اسپرم در رحم و در نتیجه شانس بارداری را افزایش می‌دهد. هدف از تحقیق حاضر مقایسه بهره‌‌وری کاتتر Masstec (تولید داخل) با نمونه وارداتی آن (PMIU) بود تا در صورت بهره‌وری مشابه، از نمونه تولید داخلی به عنوان جایگزین نمونه وارداتی استفاده گردد. روش بررسی: این مطالعه یک کارآزمایی بالینی شاهددار تصادفی از نوع مطالعه همزمان موازی بود که در بیمارستان شهید بهشتی اصفهان بین سال‌های 98-1397 انجام شد. در این مطالعه از 62 زوج نازا 30 نفر از کاتتر (Masstec)، محصول شرکت مهندسی سلامت‌یار حکیم - ایران و 32 نفر از کاتتر وارداتی (PMIU) محصول شرکت Ellios BioTek - فرانسه استفاده کردند. پیامدهای اولیه شامل مشکل در جاگذاری کاتتر، نیاز به استفاده از تناکولوم و نیاز به دیلاتاسیون سرویکس در زمان انجام IUI بود. پیامدهای ثانویه شامل وقوع بارداری شیمیایی و وقوع بارداری کلینیکی و وقوع سقط بود. داده‌ها با استفاده از آزمون‌های مجذور کای، تی و رگرسیون لجستیک مورد تجزیه و تحلیل قرار گرفت. نتایج: نتایج آزمون دقیق فیشر اختلاف معنی‌داری را در پیامدهای اولیه بین دو گروه نشان نداد (p>0/05). پیامدهای ثانویه شامل میزان بارداری شیمیایی، کلینیکی و سقط  نیز بین دو گروه اختلاف معنی داری نداشتند (p>0/05). نتیجه‌گیری: مطالعه حاضر نشان داد که اثر بخشی و عوارض جانبی کاتتر IUI تولید داخل Masstec با کاتتر PMIU تفاوتی ندارد. با توجه به کارایی و عوارض مشابه و هزینه کمتر کاتتر IUI تولید شده توسط شرکت داخلی نسبت به کاتتر وارداتی، استفاده از کاتتر داخلی برای بیماران کاندید IUI توصیه می‌شود.  </div> <div style="text-align: justify;">Introduction: Intrauterine insemination (IUI) is one of the assisted reproductive techniques, which increases the likelihood of egg being exposed to sperm in the uterus and thus the chances of pregnancy. The purpose of the present study was to compare the efficiency of the Masstec catheter (the internal product) with imported catheter (PMIU) so that, if similar, use domestic production as an alternative to imported. Methods: This study was a randomized controlled clinical trial continuous sampling method performed in Shahid Beheshti Hospital in Isfahan between 2016-2017. In this study, out of 62 infertile couples, 30 used catheters (Masstec), produced by Salamat Yar Hakim Engineering Company - Iran, and 32 imported catheters (PMIU), produced by Ellios BioTek – France. Primary outcomes included difficulty in catheter placement, the need to use a tenaculum, and the need for cervical dilatation during IUI. Secondary outcomes included chemical pregnancy, clinical pregnancy, and abortion. The data were analyzed using Chi-square, t-test and logistic regression. Results: Fisher exact test results showed no significant difference in primary outcome between the two groups (P>0.05). Secondary outcomes, including chemical, clinical and abortion rates were not significantly different between the two groups (P>0.05). Efficacy and side effects of Masstec IUI catheter were not different from PMIU catheter. Conclusion: Our study showed that the efficacy and side effects of the Masstec IUI catheter were not different from PMIU catheter.
