fa مقایسه دو روش پروستاتکتومی باز مدیفیه و برداشتن پروستات از طریق مجرای ادراری در بیماران مبتلا به هایپرپلازی خوش‌خیم پروستات; یک کارآزمایی بالینی Comparison of Modified Open Prostatectomy and Classic TURP in Patients with Benign Prostatic Hyperplasia: A Clinical Trial جراحی Surgery کارآزمایی بالینی clinical trial <div style="text-align: justify;">مقدمه: این مطالعه با هدف مقایسه نتایج حاصل از بهبودی بیماران مبتلا به هایپرپلازی خوش‌خیم پروستات(BPH) Benign prostatic hyperplasia &lrm; از طریق دو روش جراحی شامل برداشتن پروستات از طریق مجرای ادراری TURP) Transurethral resection of prostate &lrm;) و روش پروستاتکتومی باز مدیفیه (Modified Open prostatectomy) انجام گرفت. روش بررسی: در این مطالعه کارآزمایی بالینی 100 بیمار مبتلا به BPH که از سال96تا97به کلینیک اورولوژی بیمارستان شهید رهنمون مراجعه کرده بودند، بر اساس معیارهای ورود و خروج وارد مطالعه شدند و به روش تصادفی در یکی از گروه‌های جراحی به روش پروستاتکتومی باز مدیفیه و یا TURP وارد شدند. نتایج حاصل از مطالعه در هر 2 گروه به‌وسیله نرم‌افزا16  SPSS، مورد آنالیز قرار گرفت. با توجه به نرمال بودن توزیع داده‌ها از تست‌های آماری T-test و Chi-Square جهت تحلیل داده‌ها استفاده شد. نتایج: میانگین سنی درگروه OP مدیفیه، 8/35±70/36 و درگروهTURP، (8/44±69/39) سال بود. میانگین ﺟﺮیﺎن &rlm;ﺣﺪاﮐﺜﺮ ادرار(Qmax&rlm;) بعد از عمل درگروهOP مدیفیه به صورت معنی‌داری بیشتر از گروه TURP بود (0/001P<). هم‌چنین، میانگین مدت زمان عمل در گروه OP مدیفیه نیز به صورت معنی‌داری کمتر از گروه TURP بود (0/001P=).  افزایش علائم انسدادی و تحریکی 6 ماه بعد از عمل درگروه TURP از لحاظ آماری معنی‌دار بود (به ترتیب 0/009P= و0/011 P=). نتیجه‌گیری: هر چند TURP یک روش استاندارد درمان بیماران مبتلا به BPH می‌باشد، اما نتایج این مطالعه نشان می‌دهد که OP مدیفیه یک روش بی خطرتر و موثرتر با عوارض بعد از عمل کمتر می‌باشد. توصیه می‌شود OP مدیفیه به عنوان یک روش درمانی برای بیماران BPH در نظر گرفته شود.</div> <div style="text-align: justify;">Introduction: The aim of this study was to compare the results of recovery of patients with benign prostatic hyperplasia through two surgical methods, including TURP (Transurethral resection of prostate) and modified open prostatectomy (OP) was performed. Methods: In this clinical trial study, 100 patients with benign prostatic hyperplasia who were referred to the Urology Clinic of Shahid Rahnemon Hospital from 1996 to 1997 were admitted to the study on the inclusion and exclusion criteria and were randomly assigned to one of the surgical groups using the modified OP or TURP approach. The results of the study in both groups were analyzed through SPSS 16 software. Due to the normal distribution of data, T-test and Chi-Square statistical tests were applied to analyze the data. Results: The mean age of the patients undergoing modified OP surgery and the TURP patients was 70.36 ± 8.35 and 69.39 ± 8.44 years, respectively. The maximum urinary flow (Qmax) was significantly higher in the modified OP group than in the TURP group (P <0.001). In addition, the mean duration of operation in the modified OP group was significantly lower than the TURP group (P = 0.001). The increase in obstructive and excitatory symptoms in the TURP group was statistically significant 6 months after surgery (P = 0.009 and P = 0.011, respectively). Conclusion: While TURP is the standard therapy for patients with BPH&lrm;, the findings of this study indicate that modified OP is a safer and more reliable procedure with fewer postoperative complications.
