fa ساخت حلقه مخزنی با قابلیت رهایش پیوسته و طولانی‌مدت داروی لوونورژسترل جهت کاربردهای واژینال Fabrication of a Reservoir Ring for Sustained and Prolonged Release of Levonorgestrel for Vaginal Administration زنان و زایمان Obstetrics پژوهشي Original article <div style="text-align: justify;">مقدمه: بیش از یک سوم زنان در طول زندگی خود خونریزی غیرطبیعی رحمی را تجربه می‌کنند. با توجه به مشکلات قرص‌های هورمونی استفاده از حلقه‌های واژینال حاوی هورمون‌های پروژسترونی می‌تواند انتخاب مناسبی برای درمان محسوب شود. در این مطالعه طراحی و ساخت حلقه مخزنی با قابلیت رهایش هورمون لوونورژسترل مورد بررسی قرار گرفته است. روش بررسی: در مطالعه تجربی حاضر، به‌منظور ساخت حلقه‌های واژینال حاوی لوونورژسترل فایبرهای سیلیکونی زیست‌سازگار که دو سر آن با کلاهک‌های مخصوص به‌هم وصل گردیده استفاده شدند و درصدهای وزنی مختلف شامل 0/1، 0/2 و 0/4 هورمون لوونورژسترل نسبت به پارافین (به‌عنوان حامل دارو) درون فایبرها بارگذاری شدند که به‌ترتیب RSC1، RSC2 و RSC3 نام‌گذاری شدند. رهایش روزانه و تجمعی لوونورژسترل توسط دستگاه طیف‌سنج نوری اندازه‌گیری و مقایسه شده و با استفاده از نرم‌افزار آماری SPSS version 16 و آزمون تحلیل واریانس یک طرفه تحلیل شدند. نتایج: بعد از رهایش انفجاری در روز اول که به‌میزان متفاوتی برای این سه پروتوتایپ بود از روز دوم تا سی‌ام برای RSC3 و از روز چهارم تا سی‌ام برای RSC1 و RSC2 رهایش پیوسته لوونورژسترل را از پروتوتایپ‌ها داشته‌ایم که میانگین رهایش روزانه برای سه پروتوتایپ RSC1، RSC2 و  RSC3به‌ترتیب برابر 8/22 ± 24/10، 9/02 ± 41/77 و 5/07 ± 83/29 میکروگرم در روز بوده است. نتیجه‌گیری: باتوجه به یافته‌ها می‌توان حلقه‌های واژینال با قابلیت رهایش پیوسته و پایدار لوونورژسترل تا بازه 30 روزه را طراحی کرده و میزان رهایش روزانه آن‌ها را براساس بارگذاری اولیه دارو کنترل نمود. این روش می‌تواند جایگزین مناسبی برای قرص‌های هورمونی خوراکی در درمان خونریزی غیرطبیعی رحمی باشد.</div> <div style="text-align: justify;">Introduction: More than one-third of women experience abnormal uterine bleeding (AUB) during their lifetime. One of the treatment methods of this disease is hormone therapy. Vaginal rings (VRs) containing progesterone hormones could be a good choice for treatment. In this study, the design and construction of a reservoir ring with the ability to release levonorgestrel hormone has been investigated. Methods: In this experimental study, to make vaginal rings containing levonorgestrel, biocompatible silicone fibers were used, the two ends of which were connected with special caps. Different amounts of LNG including 0.1, 0.2 and 0.4 weight percent relative to paraffin were loaded into the fibers. These three prototypes were named RSC1, RSC2, and RSC3, respectively. The daily and cumulative release of LNG from these rings was measured by spectrophotometer and compared using SPSS software version 16 and analyzed by one-way ANOVA method. Results: On the first day, there was a burst release that was different for these three prototypes depending on the drug loading percentage. From the second until thirtieth day for RSC3 and from forth until thirtieth day for RSC1 and RSC2, prolonged release was achieved and average daily release for RSC1, RSC2, and RSC3 were 24.10 ± 8.22, 41.77 ± 9.02 and 83.29 ± 5.07µg, respectively. Conclusion: According to the findings, it is possible to design VRs capable of prolonged and sustained release of LNG for up to 30 days and control their daily release rate based on initial drug loading.
