fa بررسی فراوانی آنزیم های بتالاکتاماز وسیع‌الطیف در ایزوله‌های انتروباکتر آئروژنز جدا شده از موارد عفونت ادراری در شهرستان شهرکرد Prevalence of Extended-Spectrum Beta-Lactamases Enzymes in Enterobacter Aerogenes Isolated from Urinary Tract Infections in Shahrekord City میکروبیولوژی Microbiology پژوهشي Original article <div style="text-align: justify;">مقدمه: تولید آنزیم‌های بتالاکتاماز توسط باکتری‌های خانواده انتروباکتریاسه یکی از مشکلات بهداشتی و درمانی در سطح دنیاست. شیوع این آنزیم‌ها در نواحی جغرافیایی مختلف و با زمان تغییر می‌کند. با توجه به عدم مطالعه نسبت به شناسایی ژن‌های بتالاکتامازهای وسیع الطیف blaSHV, blaCTX-M, blaTEM در ایزوله‌های انتروباکتر آئروژنز جدا شده از موارد عفونت ادراری در شهرستان شهرکرد، ژن‌های مذکور از نظر فنوتیپی و ژنوتیپی بررسی گردید. روش بررسی: در این مطالعه توصیفی مقطعی، الگوی حساسیت ضد باکتریایی 50 ایزوله انتروباکتر آئروژنز به سفوتاکسیم، سفتازیدیم، سفتریکسون و سفالوتین با استفاده از روش انتشار دیسک  مورد آزمایش قرار گرفت. سپس ایزولههای مقـاوم بـه آنتی بیوتیکهای فوق به‌وسیله تست تاییدی دیسک‌های ترکیبی سفوتاکسیم-کلاولانیـک اسـید و سـفتازیدیم- کلاولانیک اسید بررسی شدند. سپس در سویه‌های مولد بتالاکتاماز وسیع‌الطیف وجود ژن‌های blaSHV, blaCTX-M, blaTEM, در حضور پرایمرهای اختصاصی بررسی شد. نتایج: از 50 ایزوله انتروباکتر آئروژنز مورد بررسی در این مطالعه 32 ایزوله (64 درصد) در بررسی فنوتیپی مولد بتالاکتامازهای وسیع‌الطیف تشخیص داده شدند که  فراوانی ژنهای blaCTX-M  ،blaTEM  و blaSHV به ترتیب 30 درصد، 20 درصد و 14 درصد گزارش گردید. در تجزیه و تحلیل آماری بین مقاومت به آنتی‌بیوتیک سفتریکسون و ژن  blaCTX-Mارتباط آماری معنی‌داری مشاهده گردید (0/05p <). نتیجه‌گیری: مطالعه حاضر حاکی از آن است که ایزوله‌های انتروباکتر آئروژنز مولد ESBL از شیوع نسبتا بالایی برخوردارند. افزایش میزان این گونه‌ها غالباً ناشی از تجویز غیر منطقی آنتیبیوتیک‌ها است که رفع این مشکل مستلزم به‌کارگیری عوامل ضد میکروبی جدید و محدود نمودن استفاده غیرضروری از عوامل ضدمیکروبی و افزایش بهره‌گیری از ابزارهای کنترل عفونت می‌باشد.</div> <div style="text-align: justify;">Introduction: Enterobacteriaceae produce the Extended-Spectrum Beta-Lactamases which is considered as an important resistant mechanism of beta-lactam antibiotics. The resistance to beta-lactam antibiotics is the main problem in the bacterial infections therapy. The prevalence of these enzymes changes in different geographical areas and with time. The present study aims to explore the frequency of the extended-spectrum blaSHV, blaCTX-M, blaTEM, in Enterobacter aerogenes isolated from urinary tract infections in Shahrekord City; the mentioned genes were investigated from the phenotypic and genotypic point of view. Methods: In this cross-sectional descriptive study, antibacterial susceptibility patterns of 50 isolates of Enterobacter aerogenes to Cefotaxim, Ceftazidim, Cefterixon and cephalotin were tested using disk diffusion (Kirby-Buer) method. In addition, confirmatory tests for detecting ESBLs phenotypes were performed using Ceftazidim-clavulanic acid and Cefotaxim- clavulanic acid combination disks. Then, the presence of blaSHV, blaCTX-M, blaTEM, genes in the broad-spectrum beta-lactamase producing strains was assessed in the presence of specific primers. Results: Out of 50 isolates of Enterobacter aerogenesis investigated in this study, 32 isolates (64%) were identified to produce broad-spectrum β-lactamases in the phenotypic study of ESBLS, and the prevalence of blaCTX-M, blaTEM and blaSHV was reported as 30%, 20% and 14%, respectively. In the statistical analysis, a statistically significant correlation was observed between resistance to the antibiotic ceftriaxone and the blaCTX-M, gene (p<0.05). Conclusion: The present study suggests that ESBL producing Enterobacter aerogenes isolates have a high prevalence.
